35 y old male , father of two children, presented with Lt scrotal enlargement
Alpha feoto nx. bHCG slightly high
U/s diffuse left testicular mass
Rt diffuse microcalcifications
CT normal

How to follow after left Radical orchiectomy
Interesting case indeed.
I would suggest a right testicular biopsy during the time of the left orchiectomy.
Thereafter, the treatment for the right testis will be decided accordingly if there is an ITGCN (monitoring, radiotherapy for the right testis vs right orchiectomy). Surely not an easy decision to make, and it should be done along with the patient, especially if the patient still wants to have children - I don't think it would be the case with this patient.
